Aging Out of Foster Care Program - Navigating the Road to Health

Navigating the Road to Health was initiated by the Nebraska Families Collaborative in partnership with Project Everlast to improve health and mental health access and outcomes for current and former youth in foster care.

What does that mean?

It means we are here to assist current foster youth in gaining the skills they will need as they move out of the foster care system. We also offer a curriculum to provide foster parents with information to teach youth the skills they will need.

Our primary objectives are to:

  • Evaluate the impact of Region 6’s TRY Team.
  • Establish medical homes with youth prior to aging out.
  • Equip youth 14-18 with their Electronic Life Records.
  • Promote the online youth and foster parent curriculums.
  • Implement Healthcare Fund for youth formerly in foster care.

  5. Upon completion of all 5 quizzes, please complete a brief survey.
