Healthcare Transition Planning with Teenage Foster Youth

The training, “Navigating the Road to Health: Healthcare Transition Planning with Teenage Foster Youth,” has been approved for 1.5 hours of in-service training for licensed foster parents. This FREE online course is recommended for foster parents that work with teens 14 to 18 years of age.

After completing this course, participants will be able to:

  • Name unique health needs of foster youth
  • Understand the importance of healthcare transition planning
  • Identify age-appropriate healthcare skills for teens
  • List health insurance options available to youth leaving foster care
  • Help youth transition to adult healthcare providers
  • Help youth create a reproductive life plan
  • Explain the importance of a Medical Power of Attorney for aging out foster youth
